Sajdah Sahw (Prostration of Forgetfulness)

Sajdah Sahw is two extra prostrations made at the end of the prayer to make up for certain mistakes made by forgetfulness. The Prophet ﷺ himself made it when he forgot in prayer. It repairs a missed wajib; it cannot repair a missed fard.

1

When it becomes necessary

In the Hanafi school Sajdah Sahw is wajib when, by mistake, you: leave out a wajib act (e.g. forget to add a surah in the first two rak'ahs of fard, forget the first qa'dah, forget Qunoot in Witr); delay a fard or wajib (e.g. pausing silently long enough to say 'Subhanallah' three times); repeat a fard such as ruku; or, as imam, recite aloud in a silent prayer or quietly in a loud one.

If a wajib is left intentionally, Sajdah Sahw does not suffice and the prayer should be repeated. If a fard (e.g. ruku, a sajdah, the final sitting) is missed, the prayer must be repeated.
2

Hanafi method: Tashahhud and one salam

In the final sitting, recite the Tashahhud only, then give one salam to the right side.

Meaning: All compliments, prayers and pure words are for Allah. Peace be upon you, O Prophet, and the mercy of Allah and His blessings. Peace be upon us and upon the righteous servants of Allah. I bear witness that there is no god but Allah, and I bear witness that Muhammad is His servant and Messenger.

Sahih al-Bukhari 1226; Sahih Muslim 572

Two sajdahs

After the salam, say 'Allahu Akbar' and make a sajdah, reciting 'Subhana Rabbiyal-A'la' three times; sit up with 'Allahu Akbar', then make the second sajdah in the same way and sit up again.

Subhana Rabbiyal-A'la.

Meaning: Glory be to my Lord, the Most High.

Complete the prayer

Now recite the Tashahhud again, then Durood Ibrahimi and a dua, and finish with salam to both sides.

Meaning: O Allah, send Your mercy upon Muhammad and the family of Muhammad, as You sent mercy upon Ibrahim and the family of Ibrahim; You are Praiseworthy, Glorious. O Allah, bless Muhammad and the family of Muhammad, as You blessed Ibrahim and the family of Ibrahim; You are Praiseworthy, Glorious.

Other schools: Shafi'is make Sajdah Sahw before the salam in all cases. Malikis make it before the salam for an omission and after the salam for an addition. Hanbalis generally make it before the salam, except in cases where the Prophet ﷺ made it after. All are based on authentic hadith.
Sahih al-Bukhari 1224; Sahih Muslim 570

Forgetting the first sitting

If you begin to stand up after the 2nd rak'ah instead of sitting: if you are still closer to sitting, sit back down (no Sajdah Sahw needed). If you have stood up fully or nearly so, do not return; continue the prayer and make Sajdah Sahw at the end.

Sunan Abi Dawud 1036; Sahih al-Bukhari 1224
6

Doubt about the number of rak'ahs

Hanafi guidance: if this is the first time in your life such doubt has happened, repeat the prayer. If it happens often, go with what you think is most likely. If you cannot decide, take the lower number, sit after every rak'ah that could be the last, and make Sajdah Sahw at the end.

Other schools generally say: build on the certain (lower) number and make Sajdah Sahw, without distinguishing the first occurrence. A follower does not make Sajdah Sahw for his own mistakes behind the imam, but follows the imam's.
